Drivers license or Identification Card
Current physical within the last year
Current 2 step ppd within the last year
Diploma, Transcript or GED
Child Abuse Clearance (we can run if you do not have one)
Social Security Card
DPW Residential testing certificate (if you do not have one you can take the test at the link provided (services.dpw.state.pa.us/pch_comptest)
Car insurance if you drive

Certified Nursing Assistants, Licensed Practical Nurses and Registered Nurses will require the following documentation:

Drivers license or Identification Card
Current physical within the last year
Current 2 step ppd within the last year
Certification or License
Child Abuse Clearance (we can run if you do not have one)
Social Security Card
Current CPR or BLS certification
Car insurance if you drive
